Prime Spine & Wellness PLLC

Website Privacy Policy

Effective date: October 7, 2026 · Last updated: October 29, 2026

This is not our Notice of Privacy Practices. This policy explains how Prime Spine & Wellness PLLC ("Prime Spine," "we," "us," or "our") handles information collected through this website, our marketing, and our text messaging program.

Your medical care and protected health information are covered by our Notice of Privacy Practices, which you can read online. You can ask for a paper copy at any time by calling (817) 346-0453. Where information you give us is protected health information, our Notice of Privacy Practices and the law control, not this policy.

1. Information we collect

Information you give us. When you fill out a form on this site, such as a contact request, an appointment request, or a referral, we receive what you type into it. That usually means your name, email address, phone number, preferred clinic, and any message you write.

Please do not include medical information in any website form, email, or text message. This website is not a secure channel for protected health information, and details typed into a web form do not receive the protections described in our Notice of Privacy Practices. To discuss your health, call us at (817) 346-0453.

Referral requests. Our website referral forms are for scheduling and routing only. Do not use them to send diagnoses, injury descriptions, medical records, or insurance identifiers. See Section 7.

Text messaging information. If you opt in to our text messages, we keep your mobile number and a record of your consent.

Health information. As a health care provider, we keep health information about the care we provide. That information is handled under HIPAA, the Texas Medical Records Privacy Act, other applicable law, and our Notice of Privacy Practices, not under this policy.

Information collected automatically. When you visit this site, we and our service providers may collect device and usage information through cookies, pixels, and similar tools. This can include your IP address, browser and device type, the pages you view, the page that sent you here, and how you interact with the site.

What we do not ask for on this website. You do not need an account. We do not ask for Social Security numbers, insurance member IDs, payment card details, or medical histories through this website.

2. How we use information

We use information to:

  • Answer your questions and requests
  • Schedule, confirm, reschedule, and follow up on appointments
  • Provide, coordinate, and bill for our services
  • Send appointment reminders, confirmations, and service messages when you have agreed to receive them
  • Send offers, wellness tips, and practice news only to people who have agreed in writing to receive them
  • Operate, secure, analyze, and improve this website
  • Measure how our advertising performs, without using protected health information
  • Meet our legal, regulatory, and professional obligations

3. Cookies, analytics, and advertising

This site uses cookies, pixels, and similar tools to keep the site working, understand how it is used, and measure and support our advertising. The companies whose tools may run on this site include:

  • GoHighLevel, the platform that hosts this website, its forms, and our text messaging. On every page it records page views and attribution data (how a visitor arrived), and it sets a first-party visitor cookie (msgsndr_id) that lasts up to one year.
  • Cloudflare, our content-delivery network. Cloudflare Web Analytics counts page views on every page without using cookies. Cloudflare and Cloudflare-protected services may set __cf_bm, a bot-protection cookie that expires after about 30 minutes of inactivity.
  • Feedspace, our patient review display service. On our referral, form, and legal pages we do not show the review widget; those pages only link to our reviews page. Our Patient Reviews page displays reviews from Feedspace when the page loads. On other pages, the review widget in our footer loads only if you click to show reviews; it may then set its own cookies and load third-party tags, including Google, Meta, and Pinterest.
  • Google (Google Analytics, Google Ads, Google Maps, and Google Fonts)
  • Meta Platforms (the Meta / Facebook pixel)
  • Our online scheduling and electronic health record system

We do not use advertising pixels on this policy, our Terms of Use, our Health Records & Complaints page, or our Notice of Privacy Practices. Our referral forms are provided by GoHighLevel and load Meta's pixel software library from Meta's servers. Loading that library contacts Meta and can disclose your IP address and browser information; your browser may also send existing Meta cookies. In our October 1, 2026 check, no Meta pixel was initialized and we observed no Meta pixel event requests from these forms. Our website platform (GoHighLevel) records page views and attribution data on every page, including those pages, and sets the first-party visitor cookie described above; Cloudflare Web Analytics counts page views on every page.

We do not send symptoms, diagnoses, patient names, appointment details, or any other protected health information to any advertising company, and we do not use that information to build advertising audiences.

Your choices. You can set your browser to refuse cookies or to warn you before one is set; some parts of the site may not work normally if you do. You can also opt out of many advertising cookies through the Digital Advertising Alliance at optout.aboutads.info and the Network Advertising Initiative at optout.networkadvertising.org. Google explains how it uses information from sites like ours at policies.google.com/technologies/partner-sites, and you can manage Meta ad settings from your Facebook or Instagram account.

4. What we do not do

  • We do not sell your personal information.
  • We do not rent, lease, or trade it.
  • We do not give it to other companies for their own marketing.
  • We do not sell or share your mobile number, your text message opt-in, or your consent records with anyone for marketing or promotional purposes.

5. How we share information

Service providers. We share information with companies that do work for us, such as website hosting, text messaging, scheduling, electronic health records, analytics, and payment processing, only as needed for them to do that work.

Legal requirements and safety. When the law, a court order, or legal process requires it, or to protect the safety and rights of our patients, staff, or others.

If the practice changes ownership. Information may transfer as part of a merger, acquisition, or sale, subject to the law. Patient records stay protected by the laws that cover them, and you keep your right to request copies of your health information.

Summary information. We may share combined or de-identified information that does not identify you.

Text messaging data is excluded. None of the sharing described in this section includes your text messaging opt-in data or consent. We do not share that information with any third party, except the service providers that operate and deliver our messaging program, and only as needed to do so.

6. Text messaging (SMS)

Our text messaging program is voluntary. Agreeing to receive texts is not a condition of receiving care or any other service.

Types of messages. With your consent, we may send (1) appointment reminders, confirmations, rescheduling notices, office updates such as holiday hours or closures, and other service messages, (2) requests for feedback about your visit, which may include a link to leave a review, and (3) offers, wellness tips, and practice news. We send feedback requests and promotional messages only to people who gave written consent with the text message consent box on our patient intake forms or, for referring offices, on our website referral forms. Consent given in person at our front desk covers service messages only. Our texts do not include clinical details about you, such as diagnoses, treatment details, or results.

What we collect and how we use it. When you opt in, we collect your mobile number, the date, time, and way you opted in, and the messages we exchange with you. We use this information only to send the messages you agreed to receive, to answer your replies, and to honor opt-outs.

Frequency and cost. Message frequency varies. Message and data rates may apply according to your wireless plan.

Opting out. Reply STOP to any message at any time. We will send one message confirming that you have opted out, then stop texting that number. To start again, reply START or check the text message consent box again on our patient intake forms or, for referring offices, our referral forms.

Help. Reply HELP to any message, call (817) 346-0453, or email [email protected].

No sharing of mobile numbers. Your mobile number, opt-in, and consent will not be sold, rented, traded, or shared with any third party or affiliate for marketing or promotional purposes. This includes our list of opted-in numbers. We share your mobile number only with the service providers that operate and deliver our messaging program, and only as needed to do so.

We will not share, sell, or rent your mobile opt-in data or consent to any third parties for marketing or promotional purposes.

Consent records. We keep a record of your opt-in and of any opt-out.

Questions and deletion requests. To ask what text messaging information we hold about you, or to ask us to remove your mobile number from our messaging list, email [email protected] or call (817) 346-0453. We may keep a record of your opt-out so that we do not text you again.

Please do not send health information by text message. Text messages are not a secure channel for protected health information. For an emergency, call 911.

Carriers. Carriers are not liable for delayed or undelivered messages.

If your number changes. Please tell us if you change or cancel a mobile number you gave us, so we do not text a number that now belongs to someone else.

Full text messaging terms are in Section 3 of our Terms of Use.

7. Referrals, attorneys, and insurers

If an attorney, medical provider, adjuster, or insurer contacts us about you, your health information, including whether you are our patient, is released only as HIPAA and our Notice of Privacy Practices allow, which generally means with your signed written authorization. An authorization covers only the information and purpose it names, and you may revoke it in writing at any time.

Our website referral forms are for scheduling only. Referring offices should send clinical details, such as records, diagnoses, and imaging, by fax to (817) 346-0967, not through the website.

8. Data security

We use administrative, technical, and physical safeguards designed to protect the information we collect.

No method of sending or storing information is completely secure, and we cannot guarantee absolute security. That is why we ask you not to send medical details through website forms, ordinary email, or text messages.

If you believe your information has been exposed, contact our Office Manager (see Section 14). If a breach affects your information, we will notify you and the appropriate authorities as the law requires.

9. Children and minor patients

This website and our text messaging program are meant for adults. We do not knowingly collect personal information directly from children under 13. If you believe a child has sent us information through this site, contact us and we will delete it.

A minor patient's health information is handled under our Notice of Privacy Practices.

10. California residents

Our Glendale, California office serves California patients. Medical information about California patients is protected by the California Confidentiality of Medical Information Act and HIPAA, and is handled under that office's notice of privacy practices.

For other personal information, California residents may ask what information we hold about them, ask us to correct or delete it, and ask us to stop using it for targeted advertising. We will not treat you differently for making a request. To make a request, email [email protected] or call (817) 346-0453.

11. Texas residents and medical records

Texas law gives consumers certain rights over personal data. Many of those rights do not apply to health information already protected by HIPAA, the Texas Medical Records Privacy Act, or our Notice of Privacy Practices. If you have a request under Texas law, email [email protected].

Medical records. To request a copy of your health records, or to file a complaint, see our Health Records & Complaints page.

This site links to services run by other companies, including our online scheduling system, Google Maps, and our patient review display service. Those companies have their own privacy policies and terms. Once you leave this site, your information is handled under their policies.

13. Changes to this policy

We may update this policy. When we do, we will change the "Last updated" date at the top of this page and post the new version here. If a change materially affects how we use information we already have, we will tell you on this page before it takes effect. Continuing to use the site or our messaging program after a change takes effect means you accept the updated policy.
